indicate what images you want to make up your package. Package 1 includes:

Your order would look like: * NOTE- Image number can be found by looking at the
1-8x10 Image 16 back of the photo.

Remember you can pick as many poses as you would like to make up your package. You can also split
your wallets into different poses but keep in mind they come printed in sheets of 4.

If you would like additional pictures to your package please include what size, quantity and Image
number. Also please indicate here if you would like to purchase your images on CD for an additional
$50.00. This is the discounted price for those ordering packages. If you just want the CD and no
package you will be charged $150.00 for the CD.

If you want any of these additional photos in Black and White please indicate that otherwise
everything will be done in color. If you would like Color Name Overlay on additional wallets please
indicate the required information as requested above for the package wallets.

All prints will be printed in color unless indicated here. Please indicate Image, size and quantity of
prints if ordering black and white.

Do you want Color Name overlay on any wallets? If so, Please indicate if you would like first Name as
well as Year or just one or the other. Indicate what color ink you would like used, Black, white or
something to match the photo. (maybe a color ink to tie in with the shirt). There is a fee of $15.00 per
a pose for Color Name Overlay. Once you pay for a pose you will not be charge again for that pose.
Example, you could order all 48 wallets and have Color Name Overlay and be charged just $15.00. If
you order 3 different images though to make up your 48 wallets you will be charge $15.00 per each
image. ($45.00)

If you want any retouching, please indicate that here and what kind of retouching. Facial (blemishes),
Stray hair or braces removal. It is a one time fee of $15.00 per pose to have retouching done. For
example, once you pay $15.00 to have braces removed for image 1 you can order as many
prints/sizes of that image.
